Kurt Oscarson Children's Organ Transplant Fund (KOCOTF)
The Kurt Oscarson Children’s Organ Transplant Fund provides financial assistance to children under the age of 18 for initial organ transplant medical expenses through an interest-free loan.
A five-member committee oversees this restricted fund, which is funded through donations on the Utah State Income Tax Form. The loan amount is determined by the Committee as well as by the availability of funds. Each recipient has a maximum lifetime benefit of $10,000. The Committee may award a lower lifetime benefit.
The Committee establishes the terms of repayment. Repayments begin monthly starting 24 months after the first payment from the KOCOTF has been made. The term of the loan is typically 60 months or 5 years.
Application Process
A Medical Questionnaire form is completed by the applicant’s health care provider.
The packet including the following forms are completed and returned to the KOCOTF. The packet must be complete prior to the case being reviewed by the KOCOTF committee.
- Application
- Personal Financial Responsibility Form (including three consecutive pay stubs or the most recent IRS tax return)
- Release of Information
- Verification of application for public assistance, i.e., Medicaid, SSI, CHIP, etc., is required. The Denial or Acceptance letter may be provided.
Review by the KOCOTF Committee of the financial and health information. A determination to grant assistance is made by the Committee. In some cases, the family is granted access to the full amount or a partial amount.
The submitted Personal Financial Responsibility (PFR) form is used to determine the financial responsibility for repayment of the interest-free loan to the KOCOTF.
The responsible party signs a contract which includes repayment guidelines.
Once the contract has been processed, transplant related expenses may be submitted to the KOCOTF for payment by the fund.
