We provide tools and support to DCFS, JJYS, HFW, and DSPD caseworkers to assist them in connecting children, youth and families to the services and resources needed to remain or return home, transition to least restrictive levels of care, and thrive in their communities.
Service Navigation
Service navigation is part of the Office of Coordinated Care and Regional Supports (CCRS). The mission of the service navigation team is to provide support to caseworkers in our DHHS sister agencies to find and utilize services to the children, youth and families DHHS serves. By providing tools and supports to our caseworkers in DCFS, JJYS, HFW, and DSPD, we support the mission of DHHS to help children and families remain together or reunify, and thrive in their homes and communities. Below are some of the tools and supports we provide to help with the navigation of services.
- 211 Contracted Provider Directory- a tool to use to find providers/services within the United Way’s database
- DHHS Service Map- a tool in map format to search for specific services based on location and other sets of criteria
- Provider Fairs- in person events that connect DHHS contracted providers, LMHA’s and other community providers, to DHHS staff
- Follow up with families referred to HFW who are found ineligible- contact and provide other referral services to help the families
Complex Care
In addition to providing the above tools and supports, the clinical social workers a part of the team provide additional support for children, youth, and families who have more complex sets of needs and circumstances. This includes gatekeeping and oversight over single case agreements with Huntsman Mental Health Institute, gatekeeping and oversight of the Archway intensive unit, helping with DCFS IOP requests, and being apart of teams/staffings for children and youth identified at IST who need additional clinical support.
