The Utah Department of Health and Human Services (DHHS) wants to know how to best support the physical and mental health of women, infants, children, and children with special health care needs. Your feedback is important and will help us know what resources are needed to help families in Utah thrive.
There are 3 different surveys you can choose from. You can take more than one survey if you want. Answer each question based on your experience and beliefs.
Community survey
Anyone is welcome to take this survey and share their ideas about how to best help Utah families.
Children with special health care needs survey
Take this survey if you have or care for a child with special health care needs.
Professional survey
Take this survey if you provide health or social services to Utah families.
Each survey takes about 20 minutes to finish. Answer the questions as best as you can. You can skip any question you are not comfortable answering or if you do not know the answer. Your responses are confidential and will be kept private. We will not ask for your name, address or other information that may identify you.
The survey is voluntary. You don't have to take it or answer every question. You will not lose services or benefits from the Utah Department of Health and Human Services if you choose not to participate. There is no penalty for not participating or answering all of the questions.
